Submit your enrollment and other benefit or absence-related forms to UNM’s Benefits & Employee Wellness Department via (choose one):

• Secure Document Submission Upload (Requires your UNM NetID and password)

• You may also submit forms to the UNM HR Service Center, located in the John & June Perovich Business Center, 1700 Lomas Blvd NE, MSC01 1220, Suite 1400, Albuquerque, NM 87131

What to expect when requesting assistance

A benefits representative will review inquiries and/or process your forms within 24-48 business hours of receipt and approval of your form.
